Olive Branch Hospice

• Attending Physician - participates in the plan of care, remains the patient's primary physician and will closely work with the case manager

• Medical Director - oversees plan of care and advises the Hospice Team

• Registered Nurse - supervises and coordinates care under the direction of the hospice physician

• Licensed Vocational Nurse - checks patient's vital signs, provides wound care treatment, verifies medications and coordinates health status of patient to RN Case Manager

• Certified Hospice Aide - provides personal care and assistance with activities of daily living

• Social Worker - provides emotional support as well as psychosocial needs of the patients and their family

• Spiritual Counselor - provides spiritual support and guidance to patients and their family

• Bereavement Support - provides continuous contact and support for families

• Volunteer - trained to provide respite companionship in order to support day-to-day activities
